J-26S XPI Rotor contacted Lid

Hello,

I had a customer contact me about an issue with their J-26S XPII.

It was running fine until yesterday, the centrifuge began to shake badly and an F2 FRS error showed. After opening the centrifuge they saw that the rotors retaining bolt head seemed to have contacted the lid and practically lathed it off.

It appears to have created a hole in the lid from the contact and released the vacuum pressure. 

My initial thought was that they hadn't seated the rotor properly or hadn't tightened the retaining bolt or both. Has anyone ever had this issue and found what the cause was (if neither of my thoughts were the cause)?
